Your Role

We are currently recruiting for an experienced Carer's Coordinator to join our multi-disciplinary mental health team.

You will work alongside mental health professionals to support the carers, friends, and family of patients with personalised support to provide a seamless support service. Specific to the role, you will provide carers, friends, and family of mental health patients, who are transitioning from a secure care setting to community with guidance, information, and support and ensure carers, friends and family are linked to appropriate services and support networks throughout their loved one’s journey to sustained wellbeing, You will ensure that they have post discharge access to community, health and social care services; supporting carers recover their quality of life.

The Carer's Coordinator will work with the following people and teams.

  • Service Managers
  • Service Users
  • Support Workers
  • Carer Coordinator

As a Carer's Coordinator I will

  • Provide information, support and guidance to carers, friends and family of patients transitioning from secure care hospital settings back into their communities across the East Midlands.
     
  • Process assessment documentation and carers support plans to assist carers in achieving their quality of life.
     
  • Provide practical and emotional help to carers on a one-to-one basis and in group situations.
     
  • Work collaboratively to aid in the timely discharge of patients ensuring carers inclusion and support throughout the process.
     
  • Liaise with colleagues to ensure friends and families of patients are provided with all relevant information and support required to maintain support for the patient.
     
  • Contribute to the identification, management, and reduction of risk in the area of responsibility.
     
  • Work closely with other members of the team ensuring appropriate care and management is provided.

Essential experience, skills required for this role:

  • QCF Diploma or NVQ level 3 in Care or Certificate in Community Mental Healthcare (or working towards) or equivalent knowledge gained through lived personal experience as a Carer.
  • Must hold a full UK driving license and have access to your own transport.
  • Possess a working knowledge of carers rights and the Care Act 2014, mental health, criminal justice and/or substance misuse.
